It's really not hard at all. I did this shot with my son's help. I set up my tripod and put the camera in bulb mode. Then I set the aperature small-ish so I would have a fairly large depth of field, then focused on a spot about 10 feet away from the camera. Turned off all the lights, triggered my shutter with a remote and used a finger laser from World Market - I think they come in a set of 5 for about $7. I wrote backwards to the camera (forward to me) and then flipped it in photoshop.
This is so cool, i want to try this, but i think its a little too hard to achieve..
